Columbine Health Systems announces Utah-based Cottonwood Healthcare will become the new management company and operator for Columbine’s skilled nursing and rehab facilities. Cottonwood will run Fort Collins-based Centre Avenue Health & Rehab, Columbine West Health & Rehabilitation, and Lemay Avenue Health and Rehabilitation, as well as Windsor-based Columbine Commons Health & Rehab and its attached Columbine Commons Assisted Living.
Only these locations will experience a management change; no other facilities, nor the ancillary businesses, will be impacted. Services will remain uninterrupted. Employees will retain jobs and seniority.
Columbine owner Bob Wilson opened his first skilled nursing facility in 1970. Wilson said, “On December 1, 2025, Cottonwood Healthcare will officially take over as the new operator. Cottonwood is well-equipped to take Columbine Health Systems to the next level. They share many of the same values as Columbine and are committed to maintaining the culture and excellence that define our company.”
