Description
The Registered Nurse is responsible for the clinical operations throughout the designated territory. This position is responsible for managing and coordinating all elements of the client’s home care needs with the multi-disciplinary team members and other health care providers. The Case Manager will oversee our skilled nursing and home health patients and maintain compliance with all Federal and State regulations governing home care.
Essential Duties and Responsibilities
- Reviews new patient referral information, completing the referral through appropriate inquiry and identification of patient needs
- Coordinates client’s home care
- Completes the initial evaluation visit and all associated documentation as required
- Communicates with physicians to ensure coordination of care in a comprehensive manner and to receive orders for services to be provided
- Regularly re-evaluating the consumer’s nursing needs
- Initiating the plan of care and necessary revisions
- Completes those services requiring substantial and specialized nursing skill
- Initiating appropriate preventative and rehabilitative nursing procedures
- Preparing clinical notes, coordinating services, and informing the physician and other personnel of changes in the consumer’s condition and needs
- Counseling the consumer and family in meeting nursing and related needs
- Provides direct clinical supervision to assigned nursing staff including LPNs and CNAs per state regulations and Ability Home Health Care policies
- Ensures competency level of clinical staff
- Demonstrates accountability for the quality of clinical services provided by team nurses, as evidenced by correct completion of all admission and revisit documentation and communication tools
- Ensures completeness of all clinical records
- Serves as a triage nurse for patients, physicians and caregivers inquiries
- Accurately identifies real/potential problems affecting the program and implements solutions with follow through and communication
- Promotes and maintains an agency environment that is in compliance with Federal, State, Medicare, and all other governing regulations and policies
- Attends case conferences, team meetings and in-services as required
- May perform other duties as required, assigned or deemed necessary by Health Care Supervisor/Administrator
- Adheres to all Ability Home Health Care policies and procedures
Qualifications/Minimum Requirements
- Current Colorado license as an RN, BSN preferred
- Current CPR certification
- Minimum of two years as a Registered Nurse preferred
- Minimum of one year of experience in home health required
- Current state driver’s license and automobile insurance
- Must have reliable transportation, as frequent local travel is a necessity
- Proficient in computer use
- Thorough knowledge of State, Federal, and ACHC regulations
- Highly motivated, self-disciplined, and have excellent communication skills
- Ability to lift 50 pounds
- Moving, lifting, or transferring of patients may involve lifting of up to 100 pounds
- Ability to exert maximal muscle force to lift, push, pull, or carry patients
- Ability to use abdominal and lower back muscles over time without fatigue
- Ability to stand for extended periods
- Ability to bend, stop, stretch, twist, sit, and reach
- Fine motor skills
